Liability Waiver & Acknowledgment *

I, the undersigned participant, hereby acknowledge and agree to the following: Voluntary Participation: I am voluntarily participating in activities organized by SWIMBLK, which may include swimming, water safety instruction, fitness activities, and related events. Assumption of Risk: I understand that swimming and physical activities carry inherent risks, including but not limited to injury, drowning, illness, and death. I assume full responsibility for any and all risks, known and unknown, associated with my participation. Medical Clearance: I certify that I am physically able to participate and have no conditions that would prevent my safe involvement. I agree to inform SWIMBLK organizers of any relevant medical conditions prior to participation. Release of Liability: I release and hold harmless SWIMBLK, its organizers, volunteers, sponsors, partners, and affiliated venues from any and all liability, claims, or causes of action resulting from my participation in club activities, whether arising from negligence or otherwise. Emergency Authorization: In the event of injury or illness, I authorize SWIMBLK to seek emergency medical treatment on my behalf. I understand I am responsible for any associated costs. Code of Conduct: I agree to respect the safety rules, fellow participants, and instructors. Disruptive, unsafe, or discriminatory behavior may result in removal from club activities.

I give permission for SWIMBLK to use photos/videos of me taken during events for marketing and promotional purposes.
